Module Description

This module allows you to add fields to a specific delivery method in the cart that the customer will need to fill out if they choose that delivery method

To configure these fields, go to the desired delivery method and add the fields with their names

Administrative part:

After purchasing and installing the module you will be able to:

Configure additional fields for each delivery method

Configure additional fields for each delivery method:

Configure additional fields for each delivery method – Site Settings → Delivery Methods → "Select the desired delivery method" → "Add additional fields and specify their names" → Save information

Client Section:

After selecting a specific delivery method, the customer will be prompted to fill out the additional fields in the cart

Ihor Lozovyi, у данного модуля нет такого функционала. Надо привлекать программиста для доработки модуля
